Workshop and Program Policy

All Access Arts policies relate to all Staff as well as Members.

Access Arts Inc. defines a Member as any person who has filled out a membership form to join the organisation.

Staff are defined as persons engaged in Office, workshop/project or program, Volunteer or Student placement work at Access Arts Inc.

Purpose:

To ensure that all Members and Staff are aware of the correct enrolment process for all workshops and programs.

Policy:

  • All Participants must become a Member.

  • All Staff and Members must be aware of the Evacuation Policy and Critical Incident Procedure.

Members:

Must complete an Enrolment Form before they start any workshop or program.

The Enrolment Form must include information regarding:

  • Disability

  • Allergies

  • Support needs

  • Behavioural disorders

  • Transport

  • Emergency numbers (including Emergency Contact person and doctor)

  • Fee to be paid

The enrolment form must also include a clause stating that 'Access Arts reserves the right to request that Members provide their own carer as a condition of their participation in Access Arts programs.'

(Arrangements for the level of payment and how to pay can be made with the Access Arts Project Coordinator or their nominee prior to commencement of the workshop or program.)

Must complete the Service User Form:

The Commonwealth State & Territory Disability Agreement (CSTDA) through Disability Services Qld requires members who experience a disability who participate in Access Arts’ workshops to complete a Service User Form.

An explanatory letter accompanies this form.

All information provided by members will remain confidential. Any member is permitted to look at their own file by contacting Access Arts.

Access Arts must:

  • Make all the relevant forms available. Such forms include:

    1. Membership Form (including Media Consent clause)

    2. Information Release Form

    3. Enrolment Form

    4. Medical Clearance Form and

    5. CSTDA Service User Form & accompanying letter

  • Must provide a list of enrolments to the relevant Staff member(s) or Facilitator on each day of the workshop or program.

  • Keep all information provided by members confidential

The Workshop/ Program Coordinator must:

  • Ensure that all Participants have filled out all the relevant forms.

  • Report on the number of participants and have an accurate list of enrolments on each day of the workshop or program.

  • Check that Members are only at the workshop/ program that they are enrolled in.

  • Report on outcomes of the workshop or program.

  • Report on any issues that arose during the workshop or program.

  • Know the location of the First Aid Kit at the workshop or program site.

  • Know the Access Arts safe combination.

  • Have an Emergency Contact list at the workshop or program site.

Workshop and Program Procedure

  • If a Participant is not a Member they must complete a Membership form before participating in a workshop or program.

  • All Members must complete an enrolment form before they start any workshop or program.

  • Members who identify as having a disability must complete a CSTDA Service User Form.

  • If the Participant/Member is unable to complete a Membership/Enrolment/ CSTDA Service User form independently, an Access Arts Inc. Staff member will assist, or his/her carer/guardian may do so.

  • All alternate arrangements for payment can be made with the Project Coordinator or their nominee prior to commencement of the workshop or project.

  • A list of enrolments will be made available to the relevant Staff member(s) or Facilitator on each day of the workshop or project.

  • On arrival, a designated Staff member will ensure that all Members have their names marked off against the enrolment list for insurance purposes and to ensure that all Members are attending the workshop or program for which they are enrolled.
